Patient Centered Medical Home/ Services Provided
We are a full service acute and preventive Patient Centered Medical Home in Midcoast Maine. We have developed a team care model to provide you with excellent, compassionate and evidence based care for newborns through end of life care. It is our privilege to provide medical care for you and your family.
Our in-office services include acute care appointments, routine physical exams, wellness visits, chronic disease management, preoperative consults, dermatologic procedures, orthopedic procedures, pap smears, contraception counseling and treatment (IUD and nexplanon placement and removal), LGBTQIA2+ informed care, end of life counseling, Osteopathic Manipulative Treatment, substance use disorder treatment, joint injections, lab services and urinalysis. We coordinate care with outside healthcare teams including medical specialists, hospice care, home health teams and surgical teams.
Well Child Checks: an opportunity to see how your child has been and prepare you all for what is to come! The goal is to check on the overall health of the child and the family environment and offer and support that may be needed.
Medicare Wellness Visits: a medicare benefit that includes ensuring you are up to date on all of the age appropriate preventive care measures. Typically a “no touch” visit, this is a time to talk about all things “wellness” and to make plans to follow up on any new concerns you may have.
Complete Physical Exams: a traditional evaluation of your well being, involving both preventive care measures and physical evaluation of your current condition
Workers’ Compensation Visits: encompass concerns related to an injury at work only, it requires paperwork and important documentation of the incident, please let us know when scheduling your appointment if it is work related.
Opioid Health Home
Opioid Health Homes (OHH) deliver integrated office-based Medication Assisted Treatment (MAT), opioid dependency counseling, and comprehensive care management for eligible MaineCare members and uninsured individuals with opioid use disorder. The OHH model utilizes team-based care to support both the individual in treatment as well as the providers delivering care.
